Making a Move
Overview
Our Making a Move project is a 12-month initiative aimed at reducing the incidence of falls in Melbourne’s western metropolitan communities. The project involves the delivery of the ‘No Falls Exercise Program’ developed by Monash University in the local government areas of Brimbank, Maribyrnong and Melton.
HealthWest’s client group includes Home and Community Care (HACC) clients aged 65-plus years who live in their own homes and have not participated in Planned Activity Groups. In particular, the projects seeks to assist people who are considered at a higher risk of falls by a health professional or older people who have been referred for assessment at a Falls and Mobility Clinic.

Objectives and Activities
- Conduct 15-week group exercise and nutrition programs for HACC clients assessed as being at risk of falls.
- Facilitate ongoing partnerships between allied health professionals and organisations delivering HACC services to older people in the prevention of falls.
