Population Health Data for HealthWest Catchment

The purpose of this report is to provide a synopsis of population health data and to create an overall picture of the HealthWest catchment. The report will assist in: catchment planning across the continuum of care; planning for Primary Care Partnership programs; member organisation planning; and other services/sectors' planning in the region.

As new data sources become available, for example the second release of Census 2011 data, HealthWest will endeavour to update this document.
