Home and Community Care (HACC) Service Development Project Manager
We are seeking an experienced Project Manager to work with existing HACC service providers to plan for the expected growth in the catchment. You will undertake the research and development work required to address potential infrastructure, service demand and workforce issues.
Join us and use your highly developed planning skills and capacity to build collaborative relationships to help shape the HACC service system in the west of Melbourne.

Health Promotion and sustainability forum
Following on from the release of the Health Promotion and Sustainability: Transitioning toward healthy and sustainable futures report, this forum will identify the next steps for integrating health promotion and sustainability. The forum will include expert speakers and case studies from community and regional health services and a Partnership Care Partnership.

Integrated Chronic Disease Management
The Australian Community Centre for Diabetes (ACCD) Knowledge Portal
The Australian Community Centre for Diabetes (ACCD) Knowledge Portal has been released and can be accessed at www.diabetesinfo.org.auThe Portal was designed and developed following extensive consultation with focus groups across community, education, research and professional staff. Its purpose is to create an interactive and culturally appropriate online knowledge bank of information, research and educational tools relating to the prevention and management of diabetes. The content and information have been reviewed and tested in CALD and Indigenous community programs across the west of Melbourne and reflect key messages underpinned by local and international research.

Veteran Best Practice seminar Mental Health and Older Veterans
This upcoming seminar is an opportunity for health providers to explore mental illness, mental health and resilience issues as they relate to the older veteran community. You will hear from clinical specialists, researchers and DVA representatives, and will be better informed about the diversity of mental health programs and services available for older veterans and their families.

The Active Service Approach in HACC – Forum on Intake
This forum forms the second of four aspart of HealthWest's collaborative project on the Active Service Model in HACC
The purpose of the forum is to:
- To provide opportunities for networking on the day.
- To exploreways to build ongoing networks with other intake workers.
- To provideinformation about commonalities and differences between intake practices withinother Local Government and Community Health
- Services in the west.
- To develop ashared understanding of the role and training needs of intake workers whenapplying Active Service Model (ASM) principles in the west.
- To exploreways of communicating with referral sources about the ASM approach

HACC intake andassessment officers, coordinators and managers of these staff.

Forum: Working Together in Child and Youth Mental Health
As part of the Child and Youth Pathways – Western Mental Health Project, a forum will be held to identify issues and gaps inmental health services for children and youth in Wyndham and Melton.
The forum will provide participants with an opportunity to:
- share knowledge, issues and insightof mental health services available for clients aged 0-25 in Wyndham and Melton
- have their say about the action areasfor the Child and Youth Pathways – Western Mental Health Project
- develop partnerships
Who should attend?
- Service providers delivering servicesto 0-25 year olds in Wyndham and Melton including managers, team leaders and clinicians from:
- mental health services
- education services (including schoolstaff engaged in welfare and support roles)
- alcohol and other drug services
- local youth services
- early years providers
- organisations working with youngpeople from CALD background
- other service organisations workingwith young people in Wyndham and Melton
