Joining up the care

HealthWest Partnership brings together 25 health care providers, community organisations and local government members in Melbourne’s west. HealthWest also seeks voluntary strategic alliances with organisations such as Victoria University and other relevant community services to strengthen coordination and input into public health policy, prevention, early intervention and chronic care initiatives.
