Ambulatory Care Sensitive Conditions

Description

The Chronic Disease Surveillance and Epidemiology Section, Department of Human Services, releases findings from the Victorian Ambulatory Care Sensitive Conditions Study. Ambulatory care sensitive conditions are those thought to be avoidable through public health interventions and early disease management, usually in primary care or other ambulatory settings. The information compiled by the Chronic Disease Surveillance and Epidemiology Section shows the rates of hospital admission for a range of ambulatory care sensitive conditions drawn from the Victorian Admitted Episodes Dataset.

Quality and Applicability

The Victorian Ambulatory Care Sensitive Conditions Study has developed a set of indicators for ambulatory care sensitive conditions – with a focus on preventable chronic diseases, national health priorities, and vaccine preventable conditions.
