Working together to connect population health, planning, prevention, early intervention and chronic care

HealthWest Partnership’s Strategic Plan 2009–2012 outlines the priorities for collaborative action and coordination in western Melbourne’s Primary Care Partnerships (PCPs) to improve the health and wellbeing of our communities. HealthWest’s strategic planning priorities to 2012 include:

  • Strengthening partnerships for improving health planning and policy, workforce capacity building, service delivery systems and community action and involvement;
  • Supporting the coordination of prevention-related initiatives and integrated health promotion across nutrition, alcohol and drugs, sexual and reproductive health, family violence and mental health; and
  • Increasing early intervention and chronic disease management planning to tackle the rising prevalence of chronic diseases, with a particular focus on diabetes.

Review of HealthWest Partnership's Strategic Plan - May 2011

As part of the Department of Health's reporting requirements, HealthWest Partnership reviewed its 2009-2012 Strategic Plan in May 2011.  This review was undertaken in collaboration with HealthWest partners and was approved by the HealthWest Board at its June 2011 meeting.
